The Stroke Action Plan for Europe (SAP-E) is the most comprehensive European strategy ever developed to improve stroke prevention, treatment and life after stroke. Launched in 2018 by the European Stroke Organisation and SAFE and updated in 2026, SAP-E sets out clear, evidence-based targets that governments, health systems and partners can use to save lives and improve outcomes across the continent.
Stroke is a leading cause of mortality, disability and long-term care costs in Europe. SAP-E addresses every stage of the stroke journey, from primary prevention and early detection, through acute care and specialised stroke units, to secondary prevention, rehabilitation, research and life after stroke. The Plan identifies measurable goals in seven key areas and provides a framework for countries to strengthen services where they are most needed.
To support progress, SAP-E is accompanied by the Stroke Services Tracker (SST), four overarching goals and a set of 13 Key Performance Indicators (KPIs). These tools help countries measure current performance, monitor developments over time and identify gaps in care. The SST gathers data from across Europe annually, allowing stakeholders to compare progress and learn from best practice. Together, SAP-E, the SST and the KPIs create a clear mechanism for accountability, improvement and shared learning.
The Plan aligns with the broader European commitment to cardiovascular health, including the EU Cardiovascular Health Plan and recent findings from the OECD’s State of Cardiovascular Health in the European Union. It emphasises evidence-based action, multidisciplinary care and tackling inequalities so that no person affected by stroke is left behind.
